All checks can be made payable to "Carlsbad Wave Soccer"

When sending in your form, please include:

1. Registration Fee
2. Copy of birth certificate IF player has not played in our league in the last 2 years
3. Your Application can not be processed until we receive all forms. 

Registration will close January 15th, 2009 

Returned checks and refunds subject to $25.00 fee.

A Valid Photo I.D. (Driver's License or Passport)
